*Each service below is supervised by a Designated Manager & Designated Coordinator. We also have Direct Support Specialists(DSP) to monitor the self-care and health care of our clients.

We offer in-home, daily support services to help disabled adults reach their potential. These services include:
THE CADI is a program that provides home and community-based services to adults with disabilities who require a specific level of care (typically provided in a nursing facility). These services are an alternative to institutionalization. The waiver helps individuals with a disability live as independently as possible in community settings. The aim of this program is to promote optimal health, independence, safety and community integration.
A web-based application tool for conducting and creating person-centered assessments and support plans. It helps people with long-term or chronic-care needs make decisions about their care. MN CHOICES includes two electronic tools which are an assessment and support plan. The MN CHOICES Assessment allows the certified assessor to create an initial assessment. The assessment process is a person-centered, face-to-face discussion with the person to gather information about their needs, including planning and referrals driven by the person’s needs, Informed decision-making, and goal development based on a person’s strengths, needs and preferences.
